Your agreement to participate in dance classes taught by Kait Dessoffy - or an approved substitute - means that you fully agree to the following: 

• I have chosen to participate in dance instruction taught by Kait Dessoffy. I acknowledge that I understand the nature of the activities that I will be participating in, and that I am in the proper physical condition and capable of participating in the activities, understanding that Kait Dessoffy is not, in any way, responsible for making such a determination. 

• In consideration of my enrollment in any dance instruction program, or activity, I assume all risks, and I understand and agree on behalf of myself, to release, hold harmless, and discharge Kait Dessoffy, and their substitute instructors, from all claims, costs, liabilities, expenses, or judgements, including attorneys’ fees and court costs for any occurrences in connection with any dance instruction or related activities, including all risks reasonably connected with such activities whether foreseen or unforeseen. 

• My checking the box at checkout acknowledging that I have read and agree to these statements is proof of my intention to execute a complete and unconditional waiver, and release all liability pursuant to the terms herein, and agreement as to all terms and conditions contained above. I am of lawful age and competent to sign this affirmation. 

• Refunds will only be issued in the event of instructor cancellation with no make-up available. In the event of cancellation, refund will be provided in full for class or session. Class transfers or make-ups are subject to space available and at the instructor's discretion
